Pharmacy Care: Use Cases for Health Information Exchange

Introduction

Pharmacies are accessible, patient-centered care destinations, yet there is a great need for stronger clinical data exchange between pharmacies and the broader healthcare ecosystem1. Pharmacists provide care services beyond simply dispensing medications. They help manage chronic conditions, deliver preventive care, respond to acute needs through point of care testing and routine prescriptions, support transitions of care, and regularly identify gaps in both medication therapy and clinical services2. Nevertheless, pharmacy teams are often siloed from other providers due to interoperability challenges, creating inefficiencies, increased costs, and missed opportunities to improve outcomes.

Health information exchanges (HIEs) and national frameworks like the Trusted Exchange Framework and Common Agreement (TEFCA) offer paths to integrate pharmacy more fully into team-based care by enabling timely sharing of clinical data, coordinated care planning, and visibility into pharmacist-led interventions. This document outlines real-world pharmacy care scenarios, highlighting where interoperability can strengthen care delivery, enhance quality, reduce unnecessary costs, and ensure pharmacists are connected, recognized, and empowered as essential members of the care team.
